Použití epidurálních kortikosteroidů v léčbě vertebrogenních onemocnění

Kortikosteroidy zmírňují bolest omezením místního zánětu a blokováním převodu nociceptivních impulzů nemyelinizovanými pomalými vlákny typu C. Základní indikací pro podání epidurálních kortikosteroidů je subakutní (týdny) nebo chronická bolest zad s kořenovou iradiací takové intenzity, která významně omezuje běžnou činnost, schopnost pracovat a kvalitu života, přičemž bolest nereaguje na konzervativní léčbu a není bezprostřední indikace k chirurgickému zákroku. Kortikosteroidy lze podat bederním, kaudálním nebo krčním epidurálním přístupem. Déledobé (měsíce až roky) zlepšení bývá při dobře zvolených indikacích pozorováno u 60-70 % pacientů. Léčba bolesti pomocí epidurálních blokád je jednou modalitou v širším spektru péče o nemocné s vertebrogenními potížemi. V České republice je tato metoda výrazně nedoceněná.
