Terapie dysfunkcí močových cest u pacientů s roztroušenou sklerózou

Roztroušená skleróza je systémové autoimunitní onemocnění postihující bílou hmotu mozku a míchy. Dochází k demyelinizaci nervových vláken a axonální ztrátě zejména v oblasti periventrikulární, v mozkovém kmeni, mozečku a postranních a zadních provazcích míchy. Ve velkém procentu případů patří do obrazu onemocnění též symptomatologie urologická ve smyslu mikčních potíží (vysoká frekvence močení, urgence, urgentní inkontinence, projevy detruzoro-sfinkterické dyssynergie), ale i závažného poškození horních močových cest. Proto je urologická terapie zaměřena jednak na zlepšení kontinence, resp. komfortu mikce a tím i kvality života, ale také na důslednou protekci horních cest močových. Autoři shrnují současné konzervativní i operační možnosti terapie mikčních symptomů s důrazem na komplexnost a individualizaci léčby.
