Stimulace nervus vagus v terapii farmakorezistentní epilepsie

Dlouhodobá stimulace nervus vagus (VNS) byla zavedena do klinické praxe jako metoda léčby inoperabilních farmakorezistentních epileptických pacientů v roce 1997, v ČR se pravidelně používá od roku 1999. V Centru pro epilepsie v Brně bylo v období I/99-XII/01 metodou VNS léčeno celkem 36 inoperabilních farmakorezistentních epileptiků (19 mužů, 17 žen) ve věku od 17 do 57 let (průměrný věk byl 34,8 roku). Hodnocení efektu VNS terapie bylo prováděno u dvou skupin pacientů: u pacientů s minimálním odstupem 2 roky (N=11) a 1 rok (N=24) po zahájení léčby. Výsledky se v obou skupinách výrazně nelišily: významné zlepšení onemocnění (redukce v počtu záchvatů 50%) bylo dva roky po operaci pozorováno u 55% pacientů (respektive 63% ve skupině minimálně 1 rok po zahájení terapie). Při hodnocení kvality života přímým dotazem pacientů bylo zjištěno zlepšení u 73% (resp. 75%) jedinců. Při analýze vlivu typu epilepsie na efekt VNS byla zjištěna vyšší účinnost stimulace u ložiskových epilepsií v porovnání s víceložiskovými.
