Autoři v příspěvku předkládají kazuistiku 54leté ženy akutně přijaté pro ischemickou cévní mozkovou příhodu vzniklou na podkladě extrakraniální disekce vnitřní karotidy. Zdůrazňují výhody ultrazvukového vyšetření magistrálních a intrakraniálních mozkových arterií při přímém průkazu disekce a patofyziologie vzniku mozkového ischemického infarktu. V diskuzi srovnávají jednotlivé diagnostické postupy karotické disekce s konstatováním jejich výhod. V závěru shrnují současné doporučované léčebné postupy léčby karotické disekce.
