Stáří a epilepsie

Vznik epilepsie ve vyšším věku je velmi častý. Její nejčastější příčinou jsou cévní onemocnění mozku. Méně běžným etiologickým faktorem jsou neurodegenerativní choroby, neurotraumata, mozkové nádory či vlivy metabolické a exotoxické. Charakteristickým klinickým projevem epilepsie ve stáří jsou parciální záchvaty s komplexní symptomatologií, s nebo bez sekundární generalizace. Poměrně častá je postiktální Toddova paréza, déle přetrvávající postiktální amence a výskyt nekonvulzivních epileptických statů. Diagnostika je méně přímočará, často není dostupná objektivní anamnéza, neopominutelným specifikem stáří je komorbidita. Významná část případů není vůbec diagnostikována a léčena. Důležité je pečlivé vyšetření všech jedinců s podezřením na epilepsii, včetně EEG (standardní, event. po spánkové deprivaci či dlouhodobé video-EEG vyšetření) a CT mozku. V terapii jsou léky první volby karbamazepin a valproát, léky druhé volby pak lamotrigin a gabapentin.
