Iatrogenní neuromuskulární poruchy

V rámci celého komplexu diagnostických a léčebných úkonů nezřídka dochází také k poškození periferních nervů, nervosvalového přenosu i k lézi myogenní. Tyto poruchy se nazývají iatrogenní, i když je způsobil nejen lékař, ale i sestra, fyzioterapeut, či jiný zdravotnický pracovník. Z hlediska mechanizmu vzniku lze rozlišovat iatrogenní neuromuskulární poruchy vzniklé nežádoucím působením podaných léků, mechanicky, na podkladě ischemie či krvácení, ale také opožděním, či zanedbáním léčebných opatření (např. rehabilitace či nepodání důležitého medikamentu), či chybným způsobem léčby. Z pohledu lokalizace iatrogenní neuromuskulární poruchy lze rozlišovat generalizovanou neuropatii, fokální neuropatie (včetně traumatických lézí nervů), poruchy nervosvalového přenosu i různé typy myogenních poruch. Samostatnou sekci tvoří také iatrogenní komplikace vzniklé při EMG vyšetření (1). V případě podezření na iatrogenní původ neuromuskulární léze je nutno prokázat vztah diagnosticko-léčebného procesu a vzniklé poruchy, tedy stanovit iatrogenitu.
