Karotická endarterektomie je považována za standardní metodu k ošetření aterosklerotické stenózy extrakraniálního úseku karotické tepny již od 50. let minulého století. Rozvoj endovaskulárních metod v 80. letech nabídl nejen nový způsob léčby stenóz mozkových tepen, ale rozšířil možnost ošetření i na další oblasti mozkové cirkulace. Podmínkou toho, aby měl nemocný z ošetření prospěch, je nízké procento periprocedurálních komplikací. K tomu je nutná správná indikace výkonu, výběr vhodné metody, pečlivá příprava nemocného, zkušenost operujícího chirurga nebo intervenčního neuroradiologa či kardiologa a optimální pooperační péče.
