Patologické procesy selární oblasti v obraze magnetické rezonance

Magnetická rezonance (MR) se stala dominantní vyšetřovací modalitou v oblasti hypofýzy. Vždy se snažíme o substrátovou diagnostiku, neboť diferenciální diagnostika patologických lézí v oblasti selární má zásadní význam pro další klinické vedení nemocného. Zatím co u hypofyzárního makroadenomu postupuje obvykle neurochirurg operačním přístupem, u prolaktinomu se postupuje ve většině případů konzervativně, nemocný je léčen agonisty D2 receptorů. Při diagnostice meningeomu je u většiny případů nezbytné provést kraniotomii. Fatální je při diagnostice záměna aneuryzmatu za makroadenom, který bychom chtěli řešit transsfenoidálním operačním přístupem. Diferenciální diagnostika je často obtížná bez informací získaných při klinickém vyšetření nemocného, anamnézy, laboratorních vyšetření (zvláště hladin hormonů), mnohdy je nutné využít další zobrazovací metody (výpočetní tomografii, angiografii, zřídka nativní rtg vyšetření).

Studie obsahuje naše zkušenosti doplněné literárními údaji zabývajícími se touto problematikou.
