Spinální epidurální lipomatóza

Spinální epidurální lipomatóza je většinou pozorována u pacientů s dlouhodobým podáváním exogenních steroidů, ale může být také přítomna při endogenní nadprodukci steroidů, při obezitě nebo jako idiopatické onemocnění. Při tomto onemocnění dochází k hromadění tukové tkáně v páteřním kanále a ke kompresi nervových struktur páteřního kanálu. Většina pacientů má projevy myelopatie, radikulární symptomy nebo syndrom kaudy ekviny. Konzervativní terapie spočívá v redukci steroidů, léčbě obezity nebo endokrinopatie, což může vést k redukci hypertrofické tukové tkáně v páteřním kanále a k ústupu příznaků komprese nervových struktur páteřního kanálu. V případě selhání nebo nemožnosti konzervativní terapie je indikována chirurgická léčba, která spočívá v laminektomii postiženého úseku páteře a odstranění tukových hmot. Naše práce se zabývá rozborem příčin onemocnění a prezentuje kazuistiku námi operovaného pacienta.
