Kazuistika pacienta s Parkinsonovou nemocí s fluktuacemi a dyskinezami

V tomto příspěvku předkládám kazuistiku 51-letého pacienta léčeného 8 let pro Parkinsonovu chorobu, z toho byl 7 let léčen levodopou v kombinaci s anticholinergiky. Vzhledem k progresi komplikujících fluktuací a dyskinez bylo nutno přistoupit k jiné strategii terapie. Byl nasazen dopaminergní agonista v kombinaci s levodopou a byla opuštěna terapie anticholinergikem. Stav byl ztížen zvýšenou odpovědí na dopaminergní medikaci se zachováním fluktuací a hyperkinez, které přetrvávaly i při nízkých dávkách levodopy, s velmi úzkým terapeutickým oknem pro levodopu. Pacient byl postupně léčen jen v monoterapii pergolidem v dávkách až 4,5 mg pergolidu denně, přičemž se fluktuace zlepšily, ještě lepšího efektu bylo dosaženo rozdělením dávek pergolidu na 4 denně. V diskuzi se zabývám teoretickými aspekty léčby dlouhodobě působícími dopaminergními agonisty a strategií léčby fluktuací a dyskinez u Parkinsonovy nemoci.
