Ketogénna diéta – alternatívna liečba farmakorezistentnej epilepsie

Farmakorezistentná epilepsia je v praxi stálym problémom. Okrem medikamentóznej liečby kombináciami antiepileptík sa používa v indikovaných prípadoch epileptochirurgické riešenie a vagová stimulácia. V poslednom desaťročí začína renesancia diétnej liečby. Už začiatkom 20. storočia sa hladovanie používalo ako jedna z možností ako prerušiť epileptický status. Neskôr sa objavila diéta, ktorej princípom bolo udržať organizmus v ketóze a tak navodiť metabolickú situáciu podobnú, ako vzniká pri hladovaní. Takýto stav sa dosahuje príjmom tukov, proteínov a cukru v presnom pomere 4:1. Diéta sa doteraz najviac používa u detí od 1 do 14 rokov. Hlavnou indikáciou je farmakorezistentná epilepsia. U polovice detí sa dosiahne redukcia záchvatov o 50 %, 20-30 % detí je celkom bez záchvatov. V prípade efektu je možná redukcia a vysadenie antiepileptickej liečby. Nežiaduce účinky sú najčastejšie zo strany gastrointestinálneho traktu, možný je vznik urolitiázy a častejšie infekčné ochorenia.
