Léčba intracerebrálního krvácení – skepse nebo optimizmus?

Významný rozvoj medicínské techniky, především v oblasti zobrazovacích metod, umožňuje v současné době zkvalitnění terapie cévních mozkových příhod. Intracerebrální krvácení je emergentní stav, který bezpodmínečně vyžaduje co nejrychlejší hospitalizaci nemocného na jednotce neurointenzívní péče, provedení urgentní diagnostiky a okamžité zahájení adekvátní terapie podle individuálního plánu. Pokrok v terapii spočívá především v poskytnutí komplexní intenzívní péče, v rasantnějším snižování vysokého krevního tlaku v akutní fázi onemocnění a hlavně v přímém zásahu do hemokoagulačních mechanismů, kde velmi slibnou možností zamezení progrese hematomu představuje léčba rekombinantním aktivovaným VII plazmatickým faktorem (rFVIIa). Léčba antiedémová i chirurgické intervence zatím nesplňují očekávání na ně kladená.
