Diagnostika pravo-levého srdečního zkratu pomocí transkraniální dopplerovské sonografie

TCD s podáním pulmolabilní echokontrastní látky umožňuje s vysokou senzitivitou detekovat pravo-levý srdeční zkrat, kterým je nejčastěji PFO. TCD umožňuje kvantifikovat míru, a tedy i riziko zkratu a dále odhadnout jeho lokalizaci (extra- nebo intrakardiální). Oproti transezofageální echokardiografii má TCD nižší specificitu, ale je méně invazivní a lépe umožňuje detekci extrakardiálních zkratů. Vyšetření je indikováno u pacientů po ischemické cévní mozkové příhodě k průkazu možné paradoxní embolizace, dále u sledování těchto pacientů po katetrizačním uzávěru PFO. Další možnou indikací je screening pacientů s migrénou s aurou a u potápěčů s projevy dekompresní nemoci.
