Možnosti mechanické rekanalizace mozkové tepny

Okluze mozkové tepny je nejčastější příčinou mozkového infarktu. Reperfúze mozku je u těchto pacientů nejúčinnější známou terapií. Po zavedení systémové trombolýzy do běžné lékařské praxe se mimo dalších farmakologických metod začínají testovat také mechanické metody rekanalizace mozkové tepny. Autoři v článku podávají souhrnný pohled jednotlivých metod jako je použití Merci retrieveru, jiných typů endovaskulárních katetrů, katetrů s laserem či ultrazvukem, primární intrakraniální angioplastiky, sonotrombolýzy a akutní desobliterace karotidy. Metody mechanické rekanalizace mozkové tepny jsou nadějnou možností terapie mozkového infarktu. Avšak výzkum je v současnosti u většiny těchto metod v 1. nebo 2. fázi klinického testování. Přes optimistické výsledky v úspěšnosti rekanalizace tepny, je nutné na potvrzení prospěchu pro pacienty vyčkat výsledků z velkých randomizovaných studií.
