Nádorové neuropatické bolesti a jejich léčení

Bolest je častou komplikací nádorového onemocnění. Její původ a klinické příznaky přesahují do mnoha odborností. Jejich léčení však většinou zůstává na bedrech onkologů, algeziologů a praktických lékařů. Tam však bývá nejednou problém v preskripci relativně drahých opioidů. Základem analgetického postupu je farmakoterapie podle žebříčku WHO (2, 8). Některé bolesti však mají svoje specifika. K těm patří neuropatické bolesti. Provází mnoho stavů způsobených nádorem, léčením i následky onemocnění a léčení. Pro léčení nádorové bolesti platí stejná pravidla jako pro nenádorové, ale diagnostika může být komplikovanější a léčení omezeno celkovým stavem pacienta. Kvalita života je určujícím momentem. K základním prostředkům patří antikonvulziva, antidepresiva a opioidy. Terapie je vždy pečlivě individualizovaná. Cílem je maximální úleva od bolesti při zachované kvalitě života, jakou dovoluje základní onemocnění.
