Antiepileptika a kojení

V současné době je obecně kojení u žen, které užívají antiepileptika, doporučováno. A to bez ohledu na to, která antiepileptika žena užívá. Dítě je antiepileptikům vystaveno po celou dobu těhotenství transplacentárním přestupem, a to ve vyšších koncentracích. Kojení má jednoznačně větší pozitivní význam než rizika spojená s další expozicí antiepileptikům. Za potenciálně nebezpečné se považuje podávání etosuximidu, fenobarbitalu a primidonu, proto je zde potřebná pečlivá klinická kontrola kojeného dítěte spojená se stanovením hladin těchto antiepileptik v séru matky, dítěte a v mléce. Vyšší plazmatické hladiny u kojených dětí byly popsány také u lamotriginu, kde by mělo být postupováno obdobně.
